If you need to reschedule or cancel your appointment, please provide at the minimum 24hr notice so we may offer the appointment time to someone else. If you provide less than 24 hours notice, you will be charged the full price of the appointment.
Insurance Coverage:
Dr Sura is an out of network provider with all insurances. While we do not work with any insurance companies directly, we are happy to provide a receipt that you may submit to your insurance company for possible reimbursement.
